Proliva360™ Terms of Service

Service Agreement

By completing payment, the client agrees to a minimum 3-month service commitment with Proliva360™. This agreement is required to activate compliance services. Enrollment into the Proliva360™ compliance system constitutes acceptance of these terms in full.

Billing Terms

The initial payment covers onboarding, compliance setup, and the first 3 months of service. After the initial term, service continues on a month-to-month basis at $99/month unless canceled in writing prior to the next billing cycle. Proliva360™ reserves the right to update pricing with 30 days' written notice.

No Refund Policy

All payments are non-refundable once onboarding and compliance setup has begun. Because compliance activation requires immediate administrative work, driver file creation, and program enrollment, fees are earned upon initiation of services. Clients who wish to discontinue service must provide written notice to stop future billing.

Services Provided

Proliva360™ provides compliance management, driver enrollment, DOT and non-DOT program setup, random consortium management, FMCSA Clearinghouse support, and ongoing monitoring services. Additional services including drug testing, alcohol screening, background checks, and physical exams are billed separately on a per-use basis.

Client Responsibilities

Clients are responsible for providing accurate and complete information during enrollment and throughout the service period. Clients must maintain compliance with all applicable DOT and FMCSA regulations, respond promptly to required testing notifications, and submit documentation as requested. Proliva360™ is not liable for violations resulting from inaccurate information provided by the client or failure to comply with testing requirements.
